On a night when both sides needed to win to stand any chance of progressing, goals from Christian Pulisic and Chukwueze handed the Serie A side a 2-1 victory which was not enough to prolong their involvement in the competition following a 1-1 draw between Borussia Dortmund and Paris Saint-Germain in Germany.
Seven-time winners Milan at least have the consolation of a Europa League berth while Eddie Howe's men, who took a 33rd-minute lead through Joelinton's piledriver in front of a crowd of 52,037 on Wednesday night (Thursday AEDT), slipped out of Europe all together after finishing bottom of Group F in their first campaign since 2002-03.
Asked what he felt he had learnt from the European campaign, Howe said: "So much, and the value of the squad first an foremost - and that's what's exposed us really, the fact we haven't had the ability to utilise the squad that we created, and that's been a big frustration for us.
"We feel like we've almost gone into games, not with one hand tied behind our back, but to a degree very limited on what we can do. That hasn't helped our situation.
"I think always you have to use disappointments and setbacks as a motivation and a learning tool, and fuel for your fire to make sure you continue to achieve."
In Dortmund, Warren Zaire-Emery scored for PSG to squeeze the French champions into the knockout stages.
Zaire-Emery fired in the equaliser five minutes after Karim Adeyemi had scored for the hosts.
PSG claimed second place to join already qualified Dortmund in the next round, with Milan finishing third and Newcastle bottom of the group.
Earlier, Manchester City youngsters Micah Hamilton and Oscar Bobb have led the English title holders to a 3-2 win against Red Star Belgrade in their final group stage game.
Debutant Hamilton made his evening even more special with a goal in the 19th minute, while Bobb, who had already played for the team in the Champions League, scored his first-ever goal in the competition in the 62nd.
Red Star halved the deficit in the 76th through Inbeom Hwang, but Kalvin Phillips, who has failed to really make an impact with City after his big move from Leeds, added a third for City from the spot in the 85th.
Aleksandar Katai's stoppage-time goal came too late for a turnaround.
"What a goal and what a game," City manager Pep Guardiola said of Hamilton at his post-match press conference.
"I am so happy for him. He is training often with us and we saw his skills one against one. He scored a fantastic goal and made a penalty."
In the other Group G game, RB Leipzig, who had also already qualified for the last 16 as runners-up behind the defending champions, defeated Young Boys 2-1.
Elsewhere, Barcelona won Group H on goal difference after going down 3-2 at Royal Antwerp, while FC Porto thrashed Shakhtar Donetsk 5-3 to finish second.
Atletico Madrid beat Lazio 2-0 in Spain to top Group E, with the Italians safe in second, while Celtic defeated Feyenoord 2-1 for their first win of the group stage.
